The Children (Leaving Care) Act 2000 places specific duties on
fostering services and their role in ensuring that young people
leaving care make a smooth transition to adult life. These duties
include advising and assisting their needs appointing personal
advisers and drawing up written plans (Pathway Plans) for every
young person leaving care.
